The 1 min wait period is so that your muscle recuperates enough to lift heavier weight. I have found that a 30 sec rest period works AWESOME! Just test it out and use what works for you though.

For the speed of the motion, I find a count of 3 on the lift, a pause, and a count of 3 on the lower works best for me to keep my form good. Try as much as possible to isolate the muscle group being worked. If any other part of you is moving or jerking around, you are going too fast.
For breathing - EXhale on the EXertion, inhale on the lower/return motion.
